We will be re-opening program dog applications soon! Right now, we are open to owner-trained dogs and there is little to no waitlist if approved. You'll need to be local enough to Bentonville, AR to meet with us regularly to train your dog if going with an owner-train program. We can do some of these visits virtually, but you will need to travel to Bentonville or Tulsa at least once every 8 weeks to check in.

📝 Step 1: Submit the Interest Application
Filling out the form below will place you on our waitlist for consideration. Applications are reviewed monthly during our board meetings.
If we believe we may have a potential match for you, we’ll reach out to move you to the next phase.
📂 Step 2: Intake Form + Required Documents
If selected to move forward, we’ll send you a detailed intake form and request a non-refundable $1,500 deposit. At this point, we will also ask for the following:

Military Veterans:

DD-214 (showing discharge status)

VA disability verification letter

Disability breakdown report

Non-Military Applicants:

Medical paperwork showing your diagnosis
(This must match the tasks or support you are requesting a service dog for)

Documents can be submitted with your application or emailed to us within 30 days. We are unable to proceed without this documentation.
🐶 Step 3: Meet & Temperament Evaluation
Once approved, we’ll schedule a phone call or in-person visit to get to know you and your needs more personally.
If you already have a dog you’d like evaluated for service work, we will conduct a temperament assessment during this step.
📋 Step 4: Deposit & Contract
You’ll receive a copy of our program contract and be asked to submit your $1,500 deposit within 60 days of approval.
If you’re unable to pay the full deposit upfront, we’re happy to discuss fundraising options to help you reach your goal.
⏳ Wait Times
Please understand that it may take up to 48 months to receive a service dog.
Each dog is carefully selected, raised, and trained to meet your unique needs—and that takes time, precision, and love.
