Service Dog
We have decided to temporarily close our program dog applications. This is so we can train and and get dogs ready for those already on our waitlist. We will be reopening program dog applications December 1st, 2024 for a limited time, so please check back! We are still accepting other applications at this time.
Applications
Below are all of our applications. We highly suggest checking out the services page to see what programs we offer before applying. Any information provided will be kept confidential. If you have any questions please reach out!
Once you submit your application we will review it within 30 days and vote on them during our monthly meetings. Non-military veteran applicants will be asked to provide a medical diagnosis letter from their doctor. Our military veteran applicants will be asked to provide a DD 214 showing their discharge status, a verification letter and a disability breakdown. You can submit these documents with your application or via email within 30 days of applying, but we can't approve your application without these documents.
After we approve your application you will receive an email or text letting you know of your approval. Then we will set up a phone call or in person visit to get to know you more and how we can help. During this we will conduct a temperament evaluation if you have a dog of your own that you'd like to possibly use. Afterwards you will be added to our waitlist.
Following our visit with you we will then go over our contract agreement with you and take the $1500 deposit which is required. If you are unable to do the full deposit we understand and can discuss fundraising options for you to raise your deposit amount.
Please know that it may take up to 48 months before receiving a service dog. It takes a lot of time to dedicate to finding the right dog that'll match what you are needing and raising them to adults and to solidify their training.
*At this time we can not train guide dogs or allergy alert dogs. We hope to be able to offer these in the future, if you are in need of either of these types of dogs please reach out to us for recommendations*
